Nurse practitioners providing women in the community who do not have a primary care provider to receive basic healthcare services including:

• medical history questionnaire
• Well Woman exams, including Pap and breast examinations
• prescriptions for Hormone Replacement Therapy (HRT) and birth control options
• notification of any abnormal tests and appropriate referral if required
• initial prenatal assessments and referrals
• insertion of intrauterine devices such as Mirena and Nova T
• ordering of mammograms or provision of number to register for the Ontario Breast Screening Program

Provides a broad range of primary health care and health promotion services

• assess and treat injuries or illness of patients enrolled with one of the team's family doctors

Network of multi-service, community-based health and resource centres

• recognizes the importance of responding to the diversity of needs within local communities and pays particular attention to those members of the community who are most vulnerable and at risk

The 13 coalition members are six Community Health Centres, six Community Resource Centres, and a Community Service Centre, they are partners with numerous not-for-profit organizations around town and have a list on their website

Network is designed to increase collaboration and partnerships related to the delivery of primary health care services in the Champlain region * The Champlain Community Health Centres (CHC) Executive Directors meet regularly to focus on strategic priorities and to build skills and connections

A collaborative effort with community partners to provide visible, accessible, and reliable health promotion and healthy living information, education, and programs to the community.

Programs and services include:

Healthy Heart Program:
Provides educational sessions to assist those with or at risk for heart disease to improve their lifestyle and prevent future illness. Eight week exercise component that provides cardio and resistance training.

Nurse Practitioner (NP):
The NP provides general primary health care services for patients of all ages, and are involved in a variety of programs

Diabetes Prevention Program:
A free program for adults who are at high risk of developing Type 2 diabetes, with the following risk factors: overweight, especially in the stomach area; high blood pressure; high cholesterol; low physical activity levels; Diabetes during pregnancy.

Breastfeeding Support Clinic:
Free Breastfeeding Support Clinic with lactation consultant held once a week.

COVID-19 Response
During the COVID-19 Pandemic, the office will be closed to the public until further notice. Appointments will be conducted virtually (by phone or video). All group programs are temporarily on hold.

Regular Service
Medical care team focused on family and community health. Coordinates patient care in order to assess and treat injuries or illness. Team includes family physicians, nurse practitioners, registered nurses, mental health therapists, respiratory therapist and dietitian.

Hope Huntsville Palliative Care:
Provides quality end-of-life care to patients and families with an advanced progressive illness

Geriatric Care:

• Provides a comprehensive assessment and assists seniors with complex health problems.
• family and caregiver support related to senior's care including: depression, dementia, confusion, memory, falls, medication problems
• has a geriatric outreach team to provide care in home

Pulmonary Rehab/Asthma/Smoking Cessation:
Offers self-management programs for people and their families who live with asthma or COPD (Chronic Obstructive Pulmonary Disease), as well as smoking cessation.

Dietitian services:
Offers assistance to patients with:

• Weight management
• Elevated cholesterol
• Diabetes prevention
• Irritable bowel disease and other digestive disorders
• Childhood nutrition
• Sport nutrition
• Healthy eating on a budget

Mental Health Therapy services:
Counselling program offers short term counselling to adults, children, and youth for issues such as:

• Anxiety
• Depression
• Grief
• Couple issues
• Stress

Primary Care services:
Registered Nurses and Registered Practical Nurses offer chronic disease management and health promotion programs in the doctor's offices of the Algonquin Family Health Team.

Harm reduction based medical and social services to people who use drugs

• specializes in integrated care, HIV and hepatitis C care, opioid treatment, mental health care, opioid use, crack cocaine and crystal meth use, and injection drug use

• All patients attending the NSM LHIN (formerly known as CCAC) community nursing clinic will be assessed and provided with individualized instruction on self care and self-management supports

Nurse Practitioner run mobile Health Hub providing services such as:

• annual health exams
• immunizations
• sexual health
• management of chronic disease
• preventative screening such as PAPs, mammograms, prostate cancer and colon screening.

Clinic will also provides medical procedures of simple sutures, dressing changes, wart treatments and routine injections as well as prescribe and dispense non-opiate medicine.

A multidisciplinary agency serving the French-speaking population and Exogamy families of the Hamilton and Niagara Region. Services offered include primary and community health care services, counselling and community initiatives, services for families with young children, services for seniors, Community Connections Mentorship Program and the Settlement Program for French speaking newcomers.
