• one-on-one in-person appointments are available and must be made in advance, walk-in appointments are not available

Primary Care Appointments

• Call 705-734-9690 to book an appointment with a Physician, Nurse Practitioner or the clinic Nurse
• for appointments with a Diabetes Educator or Physiotherapist, ask your family physician or nurse practitioner to send a referral on your behalf
• Social Work - all one-on-one social work counselling appointments will be conducted in person or via virtual appointments
• Physiotherapy - all physiotherapy appointments are available in person or via virtual appointments
• Dietitian - all appointments will be conducted in person, via phone or video conferencing
• Diabetes - all appointments will be conducted via phone or in person if requested by the patient
• Virtual Visits - some appointments are being made available through e-visits OTN or Zoom
• In person OTN visits are available by scheduling with the receptionist

Regular Service
Health centre focused on family and community health

• encourage responsibility for one's own health and that of the community through health promotion, illness prevention, chronic disease management and integrated primary health care
• Illness Prevention - involves services such as blood pressure screening, annual immunization against influenza and wellness visits
• Diabetes Management Program - information about living with diabetes, monitoring blood sugar level, food choices, exercise, pump program, medications and lifestyle choices

COVID-19 (24 Mar 2020): Due to COVID19, the PQCHC is closed until further notice, with the exception of the medical clinic. Only clients already registered are being seen at the clinic. Previously scheduled non-essential appointments will be postponed or changed to a telephone appointment. Only essential appointments will be held in clinic and future appointments will be triaged by a nurse before booking. Chiropody, lung health and diabetes teams are not running.

• must have referral from Barrie Family Health Team doctor or Nurse Practitioner and need to access two or more of their services
• help patients reduce risk and navigate to overcome barriers

Mamaway Wiidokdaadwin meaning: Everyone Helps

A non-profit, community-governed organization that provides primary health care, health promotion, illness prevention and community development services which focus on addressing and raising awareness of the broader determinants of health such as employment, education, environment, social isolation and poverty.

The interdisciplinary team includes doctors, nurse practitioners, nurses, dietitians, health promoters, community outreach workers, outreach social workers, outreach nurses, therapists, community health workers, dentist, dental hygienists, and dental assistants among other volunteer health professionals.

COVID-19 (November 17, 2021): Entrance to the Centre is restricted to people with confirmed appointments and to those in need of emergency support from Intake. Upon arrival, clients are required to clean their hands and put on surgical mask and are then screened by staff.
