Clinic provides interim primary care services for patients of all ages, and works closely with Health Care Connect to help patients find permanent providers. Provides primary and acute health care to diagnose and treat illnesses, order and interpret tests, prescribe medications. Works to support individuals and families with mental health and well-being, ensuring they have the resources they need to heal and help navigate the health care system and understand their goals of care.
Care Clinic is now also offering:
• Social Work Services: in addition to the Nurse Practitioner team. • Paediatric Emergency Diversion: Patients 18 years and younger are able to seek non-emergency care within the COHT Care Clinic, Mon-Fri by calling the clinic directly. Spots are held each day for more acute paediatric needs in an effort to divert paediatric patients from the Emergency Department when appropriate.
