Searching for Transitional Housing

Housing program geared to helping prepare youth/young adults tranistion to independent living. Programs offer skill building, mental health supports, employment search supports and referrals to partnering agencies.

Treatment Program designed for individuals with severe and/or chronic addiction problems. The program is a highly structured and intensive intervention that operates on a 21-day closed cycle. There is a dedicated aftercare component that virtually follows participants post completion on an intermittent schedule, over a 6 week period, to assist and support the individual transition from the structure of the program back into their daily lives.
Each 21-day cycle is specifically for women or men (no co-ed cycles). Participants have the option of sleeping overnight onsite for the 21-day cycle (residential stream) or returning home overnight (day-use stream).
There is a maximum of 10 clients per cycle (7 for the residential stream, 3 for the day-use stream).
Treatment participants are expected to attend all programming and participate in all activities. This includes activities such as daily chores, walks, meals, groups, individual sessions, and journals.

Housing programs include:

Supportive Housing & Homelessness Program offers assistance with finding safe, affordable housing and/or subsidies as needed. This is an internal referral program.

Housing Resource & Co-ordination program will assist individuals with a mental health concern review listings and provide support with landlords and the application processes. Subsidies available for those who qualify.

Short-Term Residential Crisis Service Program is in collaboration with the Crisis Intervention Centre and can assist with short-term crisis housing for a person with a mental illness who have come into contact with the law.

Group Home services are provided at 2 homes providing supportive programming for life skills, daily routines and healthy living, social and organized planning with the expectation of residents to participate.

24 Hour Crisis Support.

Domestic Abuse Shelter providing up to 6 week stay. Safety Plan assistance, referrals to support agencies, and legal supports.

Program providing transitional housing for homeless or at risk of homelessness. Rent will be charged acording to OW or ODSP rental allowances.

Individuals will be provided access to couselling and resources to help with final goal to access stable housing options.

Office is open to staff, clients and essential visitors only and they will be permitted to enter if they are feeling well and do not have symptoms associated with COVID-19. All persons entering are required to wear a mask and should be prepared to participate in the COVID-19 screening process.

• provides second stage accommodation for up to 12 months for women who have been residents of Lennox and Addington Interval House or survived domestic violence

COVID-19 (13 December 2021): Dawn House remains in operation. No visitors except essential services will be allowed. Needs change daily so please call ahead and speak with a staff person regarding what donations are needed.

Supportive safe transitional and permanent housing for women

The Transitional Counsellor provides service to women living in the shelter and women in the community who have experienced family violence.

The program provides

• risk assessment and safety planning
• referrals to community resources
• help with finding, securing and maintaining housing
• Support happens within a strength based approach.

Two transitions homes:

• one for 16-24 yrs old (5 bedrooms) and another for 16-19 yrs old (6 bedrooms)
• participants work with youth counsellors on accessing school, work or volunteer opportunities through the day
• during the evening, youth learn transferable life skills such as cooking, cleaning, budgeting and overall health and well-being
• affordable and inclusive program fee for a fully furnished room
• participants buy their own groceries

Sober living transitional housing for men

• 15 private bedrooms at two houses with shared kitchen, bathroom and laundry facilities

Temporary accommodations and housing outreach services for families as they transition from homelessness to a home

• capacity for 19 people

Community Residential Facility for 10 women at risk of being in conflict with the law

• currently in conflict with the law and those on conditional release
• house rules and guidelines apply
• provides programming, advocacy and counselling as well as liaison with community resources to support transition to the community

Provides rent-geared-to-income housing and support services for single youth. * Primary housing located at RISE@one4nine, a 21-unit apartment building that includes transitional and longer-term housing. * Other housing options includes medium support housing and non-staffed apartments for youth who are able to live more independently. * Case managers offer youth various support services and mentoring to ensure emotional and physical safety

Transitional Housing

In partnership withthe Government of Canada's RH-CHS Outreach Communities Initiative, the supportive housing program was developed to help foster a positive landlord-tenant relationship and create stability for people who have experienced homelessness

• leases a rental unit and supports an individual through an Occupancy Agreement until a stable housing situation has been established

Provides support in the first six months of tenancy to establish a pathway to maintaining stable housing which includes:

• financial support to get established
• community connections
• service navigation

Assists people with a history of chronic homelessness and complex needs to find and maintain housing in the community. Provides intensive case management.

• Homelessness Partnering Strategy (HPS)

Male residential facility.

• Provides accommodation with emphasis on job creation, job placement, contacts with agencies and community groups.
• Residents participate in cleaning and general maintenance of the residential property.

• Co-ed supportive housing for people diagnosed with mental health issues who require various levels of support in order to learn independent living skills and/or maintain their independent living environment
• Support is flexible and geared towards the needs of the individual. Rent is geared-to-income
