Searching for Supportive (semi-independent) Housing

Housing program geared to helping prepare youth/young adults transition to independent living. Programs offer skill building, mental health supports, employment search supports and referrals to partnering agencies.

Housing programs include:

Supportive Housing & Homelessness Program offers assistance with finding safe, affordable housing and/or subsidies as needed. This is an internal referral program.

Housing Resource & Co-ordination program will assist individuals with a mental health concern review listings and provide support with landlords and the application processes. Subsidies available for those who qualify.

Short-Term Residential Crisis Service Program is in collaboration with the Crisis Intervention Centre and can assist with short-term crisis housing for a person with a mental illness who have come into contact with the law.

Group Home services are provided at 2 homes providing supportive programming for life skills, daily routines and healthy living, social and organized planning with the expectation of residents to participate.

Support for adults with developmental disabilities who are able to live independently, but may still require guidance in areas such as finances, home management, and community connecting.

Through LifeShare, Ontario families and citizens (home providers) can offer a safe and nurturing environment for adults with developmental disabilities (home sharers).

Social Housing. Phase 1: one and two bedroom apartments, Phase 2: one bedroom apartments including meals, housekeeping and laundry, Phase 3: one bedroom and bed sitting rooms including meals, housekeeping and laundry.

A community-based recovery focused and strengths-based program, assisting individuals with a serious mental illness in supported independent living; promoting a safe and healthy environment.

This support is founded in respect, dignity, hope and collaboration. The goals are to:

• assist people to live as independently as possible
• assist people to participate in the community
• provide supports to minimize stress, manage illness and prevent crises and hospitalization
• provide education and support to enable people to develop and maintain skills and relationships

The Housing Support Program also provides Hoarding Support Services for individuals whose tenancy may be at risk due to hoarding behaviours. Supports are individualized and provided in the client's home.

The Supportive Housing Program also works in collaboration with CMHA-HKPR to provide twenty-four (24) one-bedroom subsidized units leased through private landlords. All clients in subsidized units are supported by Supportive Housing Services and Mental Health Case Management.

Provides community residences and non-profit houses to support children and adults living with developmental disabilities

Home Share Program: individuals are placed with a host family which receives remunerations, training and support from Ongwanada

• short term respite services are available for family caregivers
