Provides members of the community, especially those who experience barriers in accessing health care to primary health care services. Such barriers may include:
• Youth
• Individuals or families experiencing poverty, housing and/or food security challenges
• Individuals or families experiencing mental health and/or addiction issues
• Individuals with any disability that presents a barrier to wellness or service access
• Isolated seniors
Works with clients to connect them with other health care providers on their team when needed, such as their mental health counsellor or dietitian or connect clients to other resources in our community.
