Regular Service
Personal transportation and shuttle services in Driverseat vehicle or customer's vehicle for:

• airport service
• special events, errands, groceries, prescriptions, elderly accompaniment, medical appointments or day surgery (after surgery, will help get customer into their vehicle), and leisure
• company donates to charities

Provides service in private homes, assisted living communities, hospitals, skilled nursing facilities, and rehabilitation centers. Services include:

• Personal Care which includes assistance with bathing, dressing and grooming, and medication reminders.

  • Medical and essential travel - will drive to areas such as the GTA for essential medical appointments
  • Transportation to COVID-19 Vaccine Clinic with in Simcoe County and Muskoka, excluding Orillia and Huntsville. Call to book an appointment or book online via CT Link Transportation to Vaccine Clinics Program
    Must have a vaccine appointment booked
  • Grocery pick up and delivery
• In-person service is available by appointment only and staff are working and available to provide assistance by email or phone

Transportation:
Provides frail seniors, and adults over the age of 18 with a physical or cognitive condition needing a non-medical escort local and long distance drives to medical appointments, central Home and Community Care Support Services funded ADP's, grocery shopping, personal errands and social outings. Priority is given to clients requiring drives to chemotherapy and dialysis appointments etc and other medical appointments before errands, shopping and outings. Clients are encouraged to call as far in advance as possible to arrange transportation.

Volunteers provide a wheelchair accessible transportation service, may accommodate scooters or other mobility devices depending on size. To qualify, users must require a wheelchair for mobility and must travel with an able escort.

Transportation service that provides door-to-door assisted transportation for seniors 55 years and older and adults with disabilities who are not eligible for Wheel-Trans * The program is led by CHATS (north of Steeles) and Circle of Care (south of Steeles) with service provided by 7 community support agencies including:

Wheelchair transportation is available for clients living in South Simcoe, Bradford/West Gwillimbury as there is no public accessible transportation available

Rides can be booked up to a day in advance * to ensure ride time, try to book 30 days in advance

Non-emergency transportation to local and long-distance medical appointments and regular activities such as grocery shopping and banking

Driver assistance includes support in and out of vehicle, with groceries and some mobility equipment * escorts are recommended, where appropriate, at no extra cost

Wheelchair Accessible Transportation - For those living in Bradford, West Gwillimbury and New Tecumseth areas, or those living in York Region going to a location YRT Mobility Plus does not go

Accessible transportation for seniors, veterans using mobility aids and people with physical or mental disabilities

• vans can accommodate up to 3 wheelchairs or other mobility vehicles, such as scooters, and 3 walk-ons
• all wheelchair clients must be accompanied by caregiver
• transportation to and from appointments, school or work
• counselling, advocacy, social activities for clients

Access to Primary Care

• provides a Personal Support Worker for clients who need to attend medical appointments and have mobility or other related challenges

Accessible Vans

• wheelchair accessible vans driven by CCD staff for clients with mobility issues
• staff push client up ramp, secure wheelchair and bring down ramp
• client has to be able to wheel themselves to ramp or have a caregiver do so
• must travel with escort if cannot travel alone

Home at Last

• provides Personal Support Worker to bring client safely home from hospital if aging or has special needs and would benefit from assistance in getting settled

Serves individuals who are unable to access public transportation and have no family or friends that are able to assist with transportation.

Provides transportation for:

• medical appointments
• day programs
• dialysis
• physio appointments
• patient transfer

Service is not wheelchair accessible. If wheelchair can be folded up, it can be transported with client.

Arranges for escorted door-to-door rides to local or out of town destinations including Toronto, Brampton and Barrie for medical appointments.

Can also provide a Personal Support Worker (PSW) to accompany eligible seniors and adults with disabilities to medical appointments.
